Basic Support Services

Seed Home Health provides a range of supportive services designed to help individuals live safely and comfortably while maintaining as much independence as possible. Each service is delivered based on the person’s assessed needs and authorized supports, with a strong focus on consistency, reliability, and compassionate care.

In-Home Support Services

In-Home Support Services focus on developing and maintaining skills that promote independence within the individual’s residence. These services are appropriate for individuals who benefit from structured instruction, coaching, and reinforcement in their home environment.

Seed Home Health also offers skill-building and supportive services focused on helping individuals gain greater independence in daily living. Instruction and support are personalized to each person’s goals and delivered according to their individual support plan, with ongoing monitoring to ensure services remain effective and appropriate.

Intervention Support Services

These services focus on reducing risk, preventing escalation, and ensuring individuals receive the right level of care and supervision, while maintaining clear communication with counties, case managers, and guardians.

Residential Supports and Services

Residential Supports and Services are provided through licensed community residential settings designed to support adults who require structured supervision and consistent staffing. These programs are appropriate for individuals who benefit from predictable environments and coordinated daily support.

Seed Home Healthcare operates licensed supported living and adult foster care settings. Residential programs emphasize safety, routine, and person-centered planning, with services delivered in accordance with DHS requirements and individual support plans.

Employment Services

Employment Services support individuals in exploring, preparing for, and maintaining participation in competitive or supported employment. These services are provided in coordination with authorized funding and individualized employment goals.

Seed Home Healthcare provides employment exploration, employment development, and employment support services. Services are designed to align vocational interests with realistic opportunities while supporting skill development and long-term engagement.
