Referrals

Seed Home Healthcare accepts referrals for licensed Home and Community-Based Services provided under Minnesota Statutes Chapter 245D. Referrals are reviewed thoughtfully to ensure appropriate service matching, safety, and alignment with authorized support plans. Our intake process is designed to support timely decision-making while maintaining clarity and collaboration with referral partners.

When Seed Home Healthcare Is an Appropriate Referral

Seed Home Healthcare is a strong referral option for adults with disabilities who benefit from structured, community-based services delivered within clearly defined parameters. Our programs are most effective for individuals who require consistent supervision, predictable routines, and coordinated support to maintain stability in the community.

Referrals are particularly appropriate when counties, case managers, or guardians are seeking a provider that prioritizes realistic service matching, clear communication, and long-term placement stability. We work best when expectations are defined, services are authorized appropriately, and there is shared commitment among the care team to consistent implementation.

Services Available for Referral

Seed Home Healthcare accepts referrals for licensed services that include Basic Support Services, In-Home Support Services, Intervention Support Services, Residential Supports and Services, and Employment Services. All services are provided within the scope of DHS authorization and in accordance with individual support plans.

For detailed information about specific services, referral partners are encouraged to review the Services section of our website or contact our intake team directly.

What to Include With a Referral

To support efficient review and timely response, referrals are most helpful when they include the following information, when available:

  • The individual’s current placement and living situation
  • Requested service type
  • Funding source and county of financial responsibility
  • Relevant support plans or assessments
  • Known safety or behavioral considerations
  • Anticipated timeline for placement or service start

Providing this information allows our team to assess fit accurately and communicate next steps clearly

How Referrals Are Reviewed
